What to Bring

Whether you’re visiting for the first time or the thirty fifth, a checkup appointment goes through a fairly standard set of tests to see how your eyes are doing. Here’s a list of what to bring:

  1. All glasses you are currently wearing including prescription sunglasses, reading glasses, computer glasses and/or sports glasses
  2. Your most current contact lens prescription or boxes
  3. Your insurance card and a photo ID, such as a license, for each office visit

Filling Out Patient Info Forms

When you schedule your appointment, either online or by phone, you will have the opportunity to save time at your next office visit. When you provide your email address you will receive the initial patient paperwork to complete prior to your arrival. Complete the forms listed below prior to your visit to save even more time. They contain basic questions about your contact information, medical history, and insurance information. We take the privacy of your information very seriously, and are compliant with all HIPPA laws both online and at the office.
