LASIK: Procedure & Options

LASIK, Laser In-Situ Keratomileusis, is a form of outpatient corneal surgery in which a surgeon uses pulses of laser light, to create a thin flap of corneal tissue. This flap is raised and laid back while still attached to the corneal tissue from the exposed bed of the cornea. The amount of tissue to be removed with an excimer laser is calculated based on the pre-operative determination of the power of your eye; these measurements are usually in agreement with recent prescriptions for your glasses and/or contact lenses. The flap is replaced and within minutes natural bonding qualities hold the flap down on the cornea and allow for effective healing without the use of stitches. Usually, within a few hours, the surface epithelium (skin) of the cornea begins to grow over the cut edge of the flap to seal it into position. LASIK can be used to correct nearsightedness (myopia) with or without astigmatism and farsightedness (hyperopia).

Why is it so popular? LASIK has advantages over other vision correction procedures, including a relative lack of pain afterward and the fact that good vision usually is achieved by the very next day.

Procedure Options:

IntraLase Technology
The IntraLase Method is the 100% all-laser approach to creating your corneal flap; the thin flap of tissue that the doctor folds back in order to perform your LASIK procedure. Traditionally, doctors have used an instrument called a microkeratome for the creation of corneal flaps. The microkeratome is a hand-held blade that moves across the eye, cutting the corneal flap as it goes. While LASIK is extremely safe, if complications do occur the microkeratome is most often the cause. With the IntraLase Method, tiny pulses of light, a quadrillionth of a second each, pass harmlessly through the outer portion of your cornea and form a uniform layer of microscopic bubbles just beneath the surface of your eye. The exact dimensions of this layer of bubbles are determined by your doctor based on what’s best for your eye, and are computer controlled for maximum precision–things that are not possible with a hand-held blade. When it’s time for your LASIK treatment to be performed, your doctor easily separates the tissue where these bubbles occur and then folds it back, thus creating your corneal flap. When LASIK is complete, a flap created using the IntraLase Method is uniquely able to “lock” back into place. Your eye then begins to rapidly heal. Hundreds of thousands of procedures have been performed safely and effectively using the IntraLase Method.

CustomVue Technology Custom
Vue Individualized Laser Vision Correction was developed by VISX; recognized worldwide for bringing innovation and breakthrough technology to laser vision correction. Using WaveScanTM-based digital technology, originally developed for use in high-powered telescopes to reduce distortions when viewing distant objects in space, doctors can now capture unique imperfections in each individual's vision that could not have been measured before using standard methods.

The information captured using the WaveScan technology is transferred to the laser, providing a new level of precision and accuracy. This new level of measurement provides 25 times more precision than with standard methods used for glasses and contact lenses.

Just like a fingerprint, each person's vision is 100% unique to their eyes. Before the recent advancements in technology, doctors were only able to use standard measurement to correct vision. With the CustomVue procedure, no two "prescriptions" are identical.

CustomVue can measure and correct the unique imperfections of each individual's vision and provide them with the potential to experience better vision than is possible with glasses and contact lenses-Personal Best Vision.

iLASIK
The technology used in the iLASIK procedure is the combination of the previously mentioned IntraLase and CustomVue technology. In fact, all branches of the U.S. military and NASA recently approved LASIK for their servicemen and women, thanks to studies using iLASIK technology.

iLASIK uses computers/software to make an individual blueprint of your eyes; this blueprint is used to drive your laser correction and is what gives you a truly customized result. AS AN ALL LASER PROCEDURE a laser is used to make a thin flap in your cornea and a second laser is used to make the correction to your cornea, based on your unique vision/eye characteristics.

The iLASIK procedure EXCLUSIVELY uses the IntraLase laser; it is part of what makes iLASIK vision correction unique. Eighty-one percent of patients choose bladeless LASIK over traditional LASIK with a blade when given the choice. The vast majority of iLASIK patients will see 20/20 OR BETTER after surgery ...That could be life changing!

PRK
Photorefractive Keratectomy, or Advanced Surface Treatment (AST), is a form of outpatient corneal surgery in which a surgeon removes the front surfaces of the cornea called the epithelium, and then reshapes the corneal bed with the laser in the same way as LASIK. After the procedure you will wear a soft contact lens (bandage lens) until the epithelial layer regenerates. Healing responses vary from patient to patient, but generally take between 3-5 days. This technique is usually used for people whose cornea may be too thin to allow for the creation of the corneal flap required for LASIK. PRK is used to correct nearsightedness (myopia), farsightedness (hyperopia), and astigmatism.

Benefits of AST:

  • Since the treatment is performed on the surface of the cornea, there is no corneal flap. Thus, the structural integrity of the cornea is maintained. This is considered to be an increased safety factor over iLASIK.
  • Some patients may have corneas considered too thin to make an iLASIK flap. AST will preserve more corneal tissue since it works on the corneal surface.
  • Patients with active lifestyles such as law enforcement, military personnel and athletes are at increased risk of sustaining trauma to their eyes. The iLASIK flap could pose an unreasonalbe risk to become dislodged however there are no flaps with AST.
  • There are nerves in the deeper tissues of the cornea that help regulate tear production. AST spares these nerves and reduces the risk of dry eyes after surgery when compared with iLASIK
